To evaluate the efficacy of sildenafil (Viagra), an angiotensin II receptor blocker (AIIB), in the treatment of erectile dysfunction (ED) in men with hypertension. A total of 40 men with hypertension (hypertensive, left-sided) and 40 healthy volunteers were recruited for the study. After a 2-week wash-out period, ED was assessed at 1- and 3-month intervals using a validated visual analogue scale (VAS). The mean change from baseline in VAS at 1- and 3-month intervals was -0.5 and -0.5, respectively, with a statistical significance (P< 0.05). The VAS at 1-month intervals for ED patients showed a statistically significant reduction of 0.5% and 0.8% (p< 0.001 and P< 0.001, respectively) compared with the placebo group, and a statistical significant reduction of 2.2% and 4.7% (p< 0.001 and P< 0.001, respectively) compared with the placebo group. The mean reduction of 1-month and 3-month measurements for ED patients and placebo was 0.4% and 0.7%, respectively. These results are consistent with the effect of the antihypertensive therapy on the improvement of erectile dysfunction in hypertensive patients with ED. However, the use of sildenafil in patients with ED is not recommended due to the increased incidence of side effects, which may require a dose adjustment of the therapy. This is a very low risk, low side effect and not a very high risk of side effects in the treatment of ED in patients with hypertension.
